How to Apply for a Passport, Get Extra Pages, or Amend It

Please see the Passport Wizard at http://travel.state.gov/passport/forms/forms_847.html.  The following instructions and forms apply to both adults and children.

Application for Passport (DS-11) http://travel.state.gov/passport/forms/ds11/ds11_842.html, which requires the applicant to appear here, by appointment only:

o     You are a first-time applicant of any age;

o     Your expired passport was issued either over 15 years ago; or your last passport was issued when you were under 16; or

o     You are replacing a mutilated, lost or stolen passport.

Application for Passport Renewal (DS-82) http://travel.state.gov/passport/get/renew/renew_833.html, which allows the applicant to either appear here, by appointment only, or drop off the necessary items, 8 to 11 am, Monday to Friday, except on Indonesian/U.S. holidays:

o        Your previous U.S. passport was issued less than 15 years ago; and

o        you were over 16; and

o        you can present it to us. 

Please bring to your passport appointment or submit:

  • Completed DS-11 or DS-82, as appropriate.
  • Proof of U.S. citizenship:  old U.S. passport, U.S. birth certificate, U.S. naturalization/citizenship certificate or consular report of birth (FS-240).  Civil records must bear the original seal, stamp or signature of the official government custodian of that record.  Each U.S. state and territory keeps its own official birth and other civil records, see www.cdc.gov/nchs/howto/w2w/w2welcom.htm.  For U.S. naturalization records see http://www.uscis.gov/.  "Certified true copies," hospital or religious records, notarized, mutilated, or illegible copies are unacceptable.
  • Current ID containing personal data and photo.
  • Passport Fee. Payable only in Indonesian Rupiah cash equivalent or American Express, Diners Club, Discover, Master Card, and Visa credit cards. We cannot accept USD cash. See Current Information on Passport Fees.
  • 2 front view, identical 2x2" photos on a white background taken within 6 months.
  • For Applicants under 16, both parents or the child's legal guardian(s) must appear.  If a parent is absent, a notarized statement of consent is required from the absent parent.
  • Applicant's birth certificate showing parents' name, if the applicant is under 16, see http://travel.state.gov/passport/get/minors/minors_834.html.
  • Prepaid, self addressed courier mailer if you or a designated courier will not pick up in person.
  • If lost or stolen:  copy of police report of loss or theft, and Statement Regarding Lost or Stolen Passport (DS-64), see http://travel.state.gov/passport/forms/ds64/ds64_845.html.  Lost or stolen passports are invalid for further travel; if recovered do not use them.

Extra pages for a Passport:  By appointment only, free of charge, as long the passport is valid and legible.  Submit in person or by courier.  Extra pages generally added within an hour.


Amendments
:  We can no long amend currently valid passports.  Applicants must seek replacement passports, submit all required forms and pay all statutory fees.
